- $793,290 for lack of fall protection for New Jersey roofers.
- $501,376 for safety failures concerning protection for employees in Maine.
- $449,583 for a trench collapse in Colorado.
- $370,347 for accidental machine start in Oklahoma based mining company.
- $263,226 for failure to protect roofers in Illinois.
- $247,269 for failing to secure safety ropes on rooftops in Pennsylvania.
- $243,406 for a trench collapse in Austin.
