5. $370,347 for accidental machine start in Oklahoma based mining company.
6. $263,226 for failure to protect roofers in Illinois.
7. $247,269 for failing to secure safety ropes on rooftops in Pennsylvania.
8. $243,406 for a trench collapse in Austin.
